I am an out-of-network provider. If you would like to use your insurance benefits to pay for services, let me know, and I can provide you with the documentation you need for reimbursement.
Missed Appointments & Cancellations
Please cancel appointments within 24 hours if possible. If you wake up sick or have an emergency of some kind, of course I will understand. Choosing to miss an appointment otherwise is always an option, but late cancellations or forgotten appointments will be billed at the regular rate unless I can fill your spot.
